About: In this Workshop you will discover how to effectively conduct and review accident investigations and how to strengthen your HSE Management System based on their results. You will obtain detailed information about the investigation process from evidence gathering to its final resolution and follow up. We will overview the techniques and methods used to determine the Root Causes of incidents (from the R4K31 course) and additionally, you will learn the reporting, documentation and knowledge sharing processes. Through the systematic determination and analysis of Root Causes from a realistic Scenario, you will make recommendations and design effective remediation and corrective actions to reduce risks and build barriers and controls that prevent the incident from being repeated in the future. The course is based on Case Studies and Real Accident from several industries. On the last day each team will share their investigation results in a 30 minute presentation
