About: This Workshop covers the techniques and methods used to determine the Root Cause of any type of incidents through a systematic analysis of information. You will learn to identify the types and classification of Root Causes and the Accident Causation Levels. The course will navigate you through the Root Cause methodology in general, but with emphasis in real accidents. You will be able to identify the causal factors of accidents, including communication, training, procedures, equipment, etc. You will put in practice the elements used to adequately address all losses and / or failures, including the design of prevention and mitigation actions to avoid recurrence. Even though it is oriented to Safety, the concepts and techniques applicable to Quality and can be used for the analysis of equipment failures. Different scenarios are worked by individual teams and results are shared in a group discussion
